Keeping lead notes source-linked

Planned or learned about. Nothing has been shown working yet.
Context
A small team was bringing together approved records before a first client conversation. The same context was being collected differently by each adviser.
Approach
The team drafted a compact note with a link beside each source statement and a space for the adviser to correct it before use.
What worked
- The note made source checks part of the review.
- The adviser could edit the draft before speaking with a lead.
- The requested access stayed limited to the records needed for the note.
What failed
- The draft was not connected to the source systems.
- Some sentences still need a clear source before they can be kept.
Lessons
- A short note is safer when every claim has a visible source.
- Permission choices should be reviewed before a workflow is connected.
Limitations
- The note format was still being checked, not used as a live qualification decision.
- It did not measure whether advisers saved time.
Still done by hand
- An adviser checks each source link.
- An adviser decides what the lead note should say.
- A reviewer confirms the access scope before connection.
Evidence
A source-linked note format has been drafted, but it has not been connected to the source systems. Source checks and the qualification decision still depend on a person, and no live outcome has been shown.
Systems: source review · lead qualification
